During your initial meeting at our dental office, our dentist will thoroughly discuss with you the various treatments that we offer including those that specifically suit your needs. You will be guided through the entire process and will have any questions answered about your oral health in general.
New Patient Paperwork
If you are pleased with the initial meeting, our team will further assist you in scheduling an appointment for a formal check-up. Before the appointment, you will be required to fill out new patient forms along with providing any prior dental history/records. We suggest that you arrive at least 20 minutes prior to your scheduled appointment to ensure that you have plenty of time to fill out these forms.
The paperwork is divided into two categories and must be fully completed. One form contains your personal information such as name, age, home address, etc. In addition, you will be required to complete a medical history form. In this form, you must provide complete information regarding past medical treatments, dental problems and, if you have dental insurance, details about the policy and the insurance provider.
When filling out the forms it is important to be precise and provide all the significant details as to avoid any misunderstandings that may or may not have an impact on your future treatments.
For your convenience, our office also provides the choice of filling out the forms before coming to the clinic. This means that you may visit our website and fill out the enrollment forms online. This saves you time and ensures an efficient and effective visit. Patients can download the health history forms from our website and fill in all the required details. This information will be stored in our database and evaluated prior to your session. Once our dentist reviews your health history forms and your appointment is set, you will be ready to meet our dentist again and begin your formal dental examination.
